Type	License Type Description
Original	Applies to persons who conduct weld tests for the purpose of qualifying structural welders - physical or radiographic.
Renewal	Renewal application for persons who already hold a weld test conductor certification.
Replacement	Duplicate card for persons who already hold a weld test conductor certification.
Modification	Modification of a card for a person who already holds a weld test conductor certification to change name or address information.
