Adjustment Service Company
An adjustment service company is engaged as principal in the business of prorating the income of a debtor to the debtor's creditor or creditors, or of assuming the obligations of any debtor by purchasing the accounts the debtor may have with the debtor's several creditors, in return for which the principal receives a service charge or other consideration.  
Adult Family Home
Licensure to operate a place where three or four adults who are not related to the operator reside and receive care, treatment or services that are above the level of room and board and that may include up to seven hours per week of nursing care per resident.  
Advertising, Outdoor
License required of any person or company that erects more than two signs in Wisconsin in a calendar year.  
Aesthetician
A license required to practice the care and beautification of the skin of the human body for compensation, including but not limited to cleaning, applying cosmetics, oils lotions, clay, creams, antiseptics, powders, tonics to or massaging, stimulating, wrapping or exercising the skin of the body. No person may engage in the practice of aesthetics unless the person has received training in the areas of service provided and holds a current aesthetician license, barber or cosmetologist license, manager license, temporary permit or training permit issued by the Barbering and Cosmetology Examining Board, or is an apprentice or a student in an aesthetics or barbering or cosmetology course of instruction.  
Aesthetics Establishment
A business entity engaged in the practice of aesthetics. No person may practice aesthetics in an establishment unless the establishment is licensed.  
Aesthetics Instructor
A licensed person who is an aesthetics instructor is a person who provides practical instruction in aesthetics in a barbering & cosmetology school or in an aesthetics school licensed by the Department.  
Aesthetics School
A business entity engaged in the practice of teaching aesthetics.

Air Pollution Control Construction Permit
With some exceptions, the construction, modification, replacement or reconstruction of any equipment, process activity or other source which may result in air pollution is required to apply for and obtain an air pollution control construction permit. Examples are a printing or coating facility, foundry, a grain elevator, a gravel or stone quarry, a paper mill, a power plant, the demolition of a building, diesel cranes, air compressors and electric generators such as those used at construction sites. Sources of certain sizes and types are exempt under s. NR 407.04 from the requirement to obtain a construction permit.  
Air Pollution Control, Elective Operation Permit
With some exceptions, the operator of any equipment, process activity or other source which may result in air pollution is required to apply for and obtain an air pollution control operation permit. Examples are a printing or coating facility, a foundry, a grain elevator, a gravel or stone quarry, a paper mill, a power plant, the demolition of a building, diesel cranes, air compressors and electric generators such as those used at construction sites. Sources of certain sizes and types are exempt under s. NR 407.03 from the requirement to obtain an operation permit. An Elective Operation Permit is an operation permit for one or more points of emissions for which a permit is not required.  
Air Pollution Control, General Operation Permit
With some exceptions, the operator of any equipment, process activity or other source which may result in air pollution is required to apply for and obtain an air pollution control operation permit. Examples are a printing or coating facility, a foundry, a grain elevator, a gravel or stone quarry, a paper mill, a power plant, the demolition of a building, diesel cranes, air compressors and electric generators such as those used at construction sites. Sources of certain sizes and types are exempt under s. NR 407.03 from the requirement to obtain an operation permit. A General Operation Permit is a general permit that covers numerous similar sources.  
Air Pollution Control, Indirect Source Operation Permit
With some exceptions, the operator of any equipment, process activity or other source which may result in air pollution is required to apply for and obtain an air pollution control operation permit. Examples are a printing or coating facility, foundry, a grain elevator, a gravel or stone quarry, a paper mill, a power plant, the demolition of a building, diesel cranes, air compressors and electric generators such as those used at construction sites. Certain indirect sources are exempt under s. NR 411.04 from the requirement to obtain an operation permit. An Indirect Source Operation Permit is an operation permit applicable to certain parking lots and road or highway projects.  
Air Pollution Research and Testing Approval
These approvals are applied for under s. NR 406.04(1)(i) by individuals or companies that seek to perform research or test a new process or process modification which would not be otherwise exempt from air pollution construction permitting if implemented on a full-time basis.  
Alcohol and Other Drug Abuse (AODA) Ambulatory Detoxification Service
An ambulatory detoxification service is a medically managed or monitored structured detoxification service on an outpatient basis, delivered by a physician or other service personnel acting under the supervision of a physician.  
Alcohol and Other Drug Abuse (AODA) Day Treatment Service
A day treatment service is a medically monitored, and non-residential substance abuse treatment service which consists of regularly scheduled sessions of various modalities, such as individual and group counseling and case management, provided under the supervision of a physician. Services are provided in a scheduled number of sessions per day and week, with each patient receiving a minimum of 12 hours of counseling per week.  
Alcohol and Other Drug Abuse (AODA) Emergency Outpatient Service
An emergency outpatient service operates an emergency phone service and provides on-site crisis intervention to deal with all outpatient emergencies related to substance abuse, including socio-emotional crises, attempted suicide and family crises; provides the examination required under s. 51.45(11)(c), Wis. Stats.; and, if needed, provides or arranges for transportation of a patient to the emergency room of a general hospital for medical treatment.  
Alcohol and Other Drug Abuse (AODA) Medically Managed Inpatient Detoxification Service
A medically managed inpatient detoxification service provides 24-hour per day observation and monitoring of patients in a hospital setting, with round-the-clock nursing care, physician management and availability of all other resources of the hospital.  
Alcohol and Other Drug Abuse (AODA) Medically Managed Inpatient Treatment Service
A medically managed inpatient treatment service is operated by a general or specialty hospital, and includes 24-hour nursing care, physician management and the availability of all other resources of the hospital.  
Alcohol and Other Drug Abuse (AODA) Medically Monitored Residential Detoxification Service
A medically monitored residential detoxification service is a 24-hour per day service in a residential setting providing detoxification service and monitoring. Care is provided by a multi-disciplinary team of service personnel, including 24-hour nursing care under the supervision of a physician. Included is the provision of an examination in accordance with s. 51.45(11)(c), Wis. Stats., and transportation, if needed, to an emergency room of a general hospital for medical treatment.  
Alcohol and Other Drug Abuse (AODA) Medically Monitored Treatment Service
A medically monitored treatment service operates as a 24-hour, community-based service providing observation, monitoring and treatment by a multidisciplinary team under supervision of a physician, with a minimum of 12 hours of counseling provided per week for each patient.  
Alcohol and Other Drug Abuse (AODA) Narcotic Treatment Service for Opiate Addiction
A narcotic treatment service for opiate addiction provides for the management and rehabilitation of selected narcotic addicts through the use of methadone or other FDA-approved narcotics and a broad range of medical and psychological services, substance abuse counseling and social services.  
Alcohol and Other Drug Abuse (AODA) Outpatient Treatment Service
An outpatient treatment service is a non-residential treatment service totaling less than 12 hours of counseling per patient per week, which provides a variety of evaluation, diagnostic, crisis and treatment services relating to substance abuse to ameliorate negative symptoms and restore effective functioning. Services include individual counseling and intervention and may include group therapy and referral to non-substance abuse services that may occur over an extended period.  
Alcohol and Other Drug Abuse (AODA) Prevention Service
A prevention service makes use of universal, selective and indicated prevention measures described in appendix A. Preventive interventions may be focused on reducing behaviors and actions that increase the risk of abusing substances or being affected by another person's substance abuse.  
Alcohol and Other Drug Abuse (AODA) Residential Intoxication Monitoring Service
A residential intoxication monitoring service provides 24-hour per day observation by staff to monitor the safe resolution of alcohol or sedative intoxication and to monitor for the development of alcohol withdrawal for intoxicated patients who are not in need of emergency medical or psychological care.  
Alcohol and Other Drug Abuse (AODA) Transitional Residential Treatment Service
A transitional residential treatment service is a clinically supervised, peer-supported therapeutic environment with clinical involvement.  
Alcohol Test Analyst
Permit to conduct alcohol analysis by individuals.  
All Terrain Vehicle (ATV) Commercial Dealer Registration
Every person who is an ATV manufacturer, dealer, distributor, renter or any combination thereof, engaged in business in this state shall register with the Department and obtain a commercial ATV certificate.  
All Terrain Vehicle (ATV) Registration
Public Use: An ATV used on marked ATV trails, frozen waters, or any lands you have permission to operate on must be registered for public use.

Private Use: used on land owned by the applicant or an immediate family member; agricultural designation on Private Use Type allows operation for agricultulral purposes.

Definition of an ATV: All-terrain vehicle" means an engine-driven device which:

1. Has a net weight of 900 pounds or less,
2. Has a width of 48 inches or less,
3. Is equipped with a seat designed to be straddled by the operator and
4. Is designed to travel on 3 or more low-pressure tires. A low-pressure tire is a tire which has a minimum width of 6 inches, which is designed to be mounted on a rim with a maximum diameter of 12 inches and which is designed to be inflated with an operating pressure not to exceed 6 pounds per square inch as recommended by the manufacturer.

(Six wheelers must be registered if the unit meets all of the definition.)

All Terrain Vehicle (ATV), Municipal Registration
ATV’s owned by the United States, another state or a political subdivision thereof shall either register their ATV with the Department or display their organization name in a visible manner on the exterior of the ATV.  
All Terrain Vehicle (ATV), Student Safety Certification
Certificate granted to individuals who complete the ATV safety course, which covers the principles of ATV safety. The purpose of the program is to reduce the potential for ATV accidents; to reduce the potential for conflict between ATV riders, landowners, and other resource users; and to promote safe, responsible, and ethical use of the environment and natural resources.  
 
All Terrain Vehicle Safety Instructor
Certificate available for people who have an interest in recreational safety and who are willing to donate a few hours of their time each year to teach safety education classes in their community. The Department of Natural Resources trains and certifies volunteers to teach about all-terrain vehicles and provides instructors with class materials and supplies. Becoming a volunteer instructor is a great way to help save lives, prevent injuries, meet people in your community, and positively influence the attitudes and actions of other resource users.  
All-terrain Vehicle Trail Pass

Non-Resident All-Terrain Vehicle (ATV) Trail Pass - ATVs operated on public trails need to display valid Wisconsin Public Use ATV registration or a valid NonResident ATV Trail Pass.

ATVs in Wisconsin for more than 15 consecutive days are required to be registered in Wisconsin.

There are no requirements to show proof of residency for the ATV or the purchaser. Passes are not assigned or designated to a specific ATV or person. Passes are not transferable from one ATV to another.

 
Ambulance Service
A business engaged in of transporting sick, disabled or injured individuals by ambulance (including air ambulance) to or from facilities or institutions providing health services.  
Ambulatory Surgical Center - Certification
Certification for Medicare/Medicaid payments to a place that provides day surgery services to persons not requiring hospitalization.  
Amusement Ride Registration and Certification
Registration and certification required before an amusement ride can be opened to the public. Certification is based on whether the ride meets standards of design, construction, and operation.  
Animal Damage Claims Enrollment
Wildlife Damage Abatement and Claims Program which provides compensation for agricultural damage caused by deer, bear, turkey, or geese.  
Animal Damage Shooting Permit
Wildlife Damage Abatement and Claims Program which requires removal of wildlife causing damage to agriculture.  
Animal Dealer License
License required for persons who, as principal or agent, deal in the business of buying livestock for resale or for slaughter, sale, or exchange, or who rent bulls or lease livestock. Does not include employees of animal dealers, or operators of farms who sell livestock if the operator keeps livestock solely for dairy, breeding, or feeding purposes and who are not otherwise engaged in buying for resale, slaughter, sale, or exchange.  
Animal Food Processor License

A license required for a person or business who processes dead animals into animal food.

 
Animal Herd Disease-Free Certificates
Certification received from the Department of Agriculture, Trade and Consumer Protection (DATCP) through testing performed by an accredited veterinarian stating that herds (cattle, swine, goats, sheep, deer) test negative for animal diseases such as pseudorabies, brucellosis, brucella ovis, tuberculosis, and chronic wasting disease (CWD). Certification facilitates the sale and transfer of these animals.  
Animal Import Permit: Cattle and Bison
Permit may be required to import cattle or bison. Permit requirement is intended to prevent import of diseased animals.  
Animal Import Permit: Circus, Rodeo, Racing and Menagerie
Permit required to import circus, rodeo, racing or menagerie animals. Permit requirement is intended to prevent import of diseased animals.  
Animal Import Permit: Llamas and Exotic Ruminants
Permit required to import a llama or other exotic hoofed animal into this state. Permit requirement is intended to prevent import of diseased animals.  
Animal Manure Import Permit
A permit must be obtained to import manure from slaughter plants or stock yards. Permit requirement is intended to prevent spread of disease organisms.  
Animal Market License
An annual license is required to operate an animal market. (See s. 95.68, Stats.) A market operator must also register livestock vehicles used by the market operator that physically carry animals. Livestock includes bovine animals, sheep, goats, swine & equine animals.  
Animal Trucker License
License required for anyone engaged for hire in the business of transporting livestock in an animal transport vehicle, except employees of such businesses.  
Animal Vaccines: Permit to Use or Distribute
Permit needed to distribute or use live vaccines of hog cholera, anthrax or swine erysipelas.  
Animals Diseased: Permit to Move
Permit is required to move any animal infected with a contagious or infectious disease.  
Annual Burning Permit form 9400-530
This annual permit grants the individual holder the right to burn small amounts of dry leaves, plant clippings, brush, paper, cardboard and clean untreated-unpainted wood in a burn barrel as long as weather conditions do not pose a fire hazard. Note: small businesses, commercial enterprises, and industries may not use burn barrels or engage in other kinds of open burning for any waste generated by the businesses.

Appraiser, Real Estate
Certification required for a person to perform in the practice of real estate appraisal.  
Approved Import Feed Lot
Voluntary registration. Annual permit designating feed lot as an "approved feed lot" for import purposes. Feeder cattle imported to an approved feed lot are exempt from some import restrictions.  
Aquatic Plant Management NR 107 Permit
Any person or lake organization or local unit of government must obtain a permit from a regional office of the DNR to use chemicals to control or treat aquatic plants or other aquatic organisms in the waters of the state. Unless a person has a valid aquatic plant management permit issued under Wisconsin Administrative Code NR 107, no person may use chemicals for the management of aquatic plants or control of other aquatic organisms in waters of the state.  
Aquatic Plant Management NR 109 Permit
Any person or lake organization or local unit of government must obtain a permit from a regional office of the DNR to control or remove aquatic plants or introduce nonnative aquatic plants from the waters of the state. Waivers from the permit requirements include: 1) manual or mechanical control from lakes less than 10 acres entirely confined on the property of one property owner; 2) manual removal of nonnative and invasive plants, 3) manual removal of aquatic plants by riparian owner within a 30 foot wide area around the pier; 4) manual or mechanical control of purple loosestrife. Unless a person has a valid aquatic plant management permit issued under Wisconsin Administrative Code NR 109, no person may do any of the following: 1) Introduce nonnative aquatic plants into waters of the state; 2) Manually remove aquatic plants from navigable waters (waiver for riparian removal of invasive plants, aquatic plants on stream beds or aquatic plants around piers for recreation and navigation); 3) Control or remove aquatic plants in navigable waters by mechanical means.

Registration required of a person to perform to offer a professional service, such as consultation, investigation, evaluation, planning, architectural and structural design, or responsible supervision of construction, in connection with the construction of any private or public buildings, structures, projects, or the equipment thereof, or addition to or alterations thereof, in which the public welfare or the safeguarding of life, health or property is concerned or involved.  
Architectural, Engineering or Designer of Engineering Systems, Corporation Certificate
A business entity engaged in the practice of architecture, professional engineering, or designing. A corporate certificate of authorization shall be issued to a corporation that practices architecture, professional engineering, or designing of engineering systems.  
Art Therapist
Certificate required for a person to perform specialized, professional and psychotherapeutic use of art media, images, the creative art process, and client responses to the created art production as reflections of an individual's development, abilities, personality, interests, concerns, and conflicts.  
Asbestos Company
Certified entity that performs, supervises, or offers to perform or supervise regulated asbestos abatement or asbestos management activities.  
Asbestos Demolition and Renovation, Notification and Application for Permit Exemption, Form 4500-113
Notification form for regulated demolition, fire training burns, and renovations involving regulated amounts of asbestos. Applies to owners and operators of regulated projects, including demolitions, fire training burns, and renovations affecting greater than 160 square feet, 260 linear feet, or 35 cubic feet of Regulated Asbestos Containing Material. Notification must be postmarked 10 working days prior to the project start date.

Asbestos Inspector
A person trained and certifed to determine the presence and location of, and assess the condition of, friable or non-friable asbestos containing materials (ACM) or suspected ACM by visual examination and by collecting samples of the materials.  
Asbestos Management Planner
A person trained and certified to develop a plan to manage asbestos in a building and make recommendations for the best response actions to be taken to manage asbestos containing material (ACM) in the building.  
Asbestos Principal Instructor
Approval for a person who is certified in the appropriate discipline or disciplines and trained in the principles and methods of adult learning, with training and experience in the topics he or she will teach and teaches more than one topical area in an accredited asbestos training course.  
Asbestos Project Designer

A person trained and certified to plan and design asbestos abatement projects or operations and maintenance (O&M) activities.

 
Asbestos Supervisor
A person trained and certified to conduct or oversee  a regulated asbestos abatement activity, or function as an air sampling professional, and has the authority to require changes in performance practices or to halt a project.  
Asbestos Training Course Accreditation
The accreditation of a training course for a specific job classification on the basis of its compliance with subchapter III of Chapter DHS 159, WI Admin Code.  
Asbestos Worker
A person trainined and certified to perform asbestos abatement activities or under the direct supervision of a certified supervisor, including but not limited to setting up containment, repairs, removal, encapsulation, enclosure, loading out or disposal of asbestos containing materials (ACM) from a building.  
Athlete Agent

This license goes into effect on October 1, 2004.

Athletic Trainer
Certificate required for a person to work as an athletic trainer. Education to include a 4-year degree and certification by NATABOC.  
Attorney (Certification for Admission to the Bar)
Certification for those seeking to practice law in Wisconsin. Lawyers/Attorneys seeking to practice in the State must be assessed by the Board of Bar Examiners, which evaluates the skills, character, and fitness of lawyers.  
Auction Company
A business entity where a person who manages an auction or who has primary responsibility for handling sales proceeds, down payments, earnest money deposits or other trust funds received by an auctioneer, the person's principal or any other person at or as a result of an auction.  
Auctioneer
Registration required for a person to perform the calling for and the recognition and acceptance of offers for the purchase of goods or real estate at an auction.  
Audiologist
License required for a person to be able to apply principles, methods or procedures of prevention, identification, evaluation, consultation, intervention, instruction or research related to hearing, vestibular function, or any abnormal condition related to tinnitus, auditory sensitivity, acuity, function or processing, speech, language or other aberrant behavior resulting from hearing loss.  
Authentication and Apostille Certificate
Certificate that certifies the qualifications of a notary public or public official who has signed a document in their official capacity. This certificate is often required when a document is destined for a foreign country.  
Authorization to Construct Facilities
To authorize public utilities to proceed with construction of proposed utility facilities at a specified cost.  
Authorized/Accredited Reinsurers
Registration required for an insurer to act as an authorized/accredited reinsurer.  
Bait Dealer
Issued to Wisconsin residents who want to sell frogs, crayfish and minnows for bait (not needed to sell worms).  
Barbering and Cosmetology Apprentice
An apprentice is a person who is contracted to an owner of an establishment or his/her designated agent, who is responsible for the supervision and training of the person in all branches of practical work and all subjects required to be taught in schools of barbering or cosmetology. Must become licensed prior to practicing as a barber and cosmetologist.   
Barbering and Cosmetology Establishment
A business entity engaged in the practice of barbering or cosmetology. No person may practice barbering or cosmetology in an establishment unless the establishment is licensed.  
Barbering and Cosmetology Instructor
License required for a person to perform by providing practical instruction in barbering and cosmetology in a school licensed by the Department.  
Barbering and Cosmetology Manager License
License required for a person to practice barbering or cosmetology and to be responsible for supervising and managing the operation of an establishment.  
Barbering and Cosmetology Practitioner License
License required for a person to practice barbering or cosmetology.  
Barbering and Cosmetology School
A business entity engaged in the teaching of barbering or cosmetology.  
Barge Fleeting Permit - Waterway & Wetland Permit
A permit is required to establish and operate a barge fleeting site on the Mississippi River or its tributaries. A permit or permits are required for activities associated with establishing and operating a barge fleeting site on the Mississippi River or its tributaries. Regulated activities include but are not limited to, long-term docking of barges, placement of mooring or docking structures, and dredging. Anyone who wants to establish and operate a barge fleeting site should contact DNR for further information.  
Bed and Breakfast Establishment
A permit to operate any place of lodging that provides 8 or fewer rooms for rent to no more than a total of 20 tourists for more than 10 nights in a 12-month period, is the owner's personal residence, is occupied by the owner at the time of rental, and in which the only meal served to guests is breakfast.  
Bingo License
A bingo license allows a charitable organization to hold an unlimited number of bingo occasions per year. There is no limit on the number of games played at a bingo occasion. However, only $1,000.00 in prizes may be given at any one bingo occasion sponsored by a licensed organization.  
Bingo Supplier License
Any person intending to sell or distribute bingo supplies or equipment to a licensed organization shall apply to the department for a supplier's license.  
Bird Hunting Preserve
If non-native game birds are to be purchased, possessed and released into the wild for commercial hunting, a bird hunting preserve license is required.
A bird hunting preserve license allows you to possess, propagate, stock, release and hunt the species of game bird approved and listed on the license. This is a commercial license, which also allows the holder to purchase and sell any of the species listed on the license and to charge others to hunt or take released birds.

There are 2 types of bird hunting preserve licenses, a Class A bird hunting preserve license and a Class B bird hunting preserve license.

Boat Registration
Registration is required for use of any motorized boat used in Wisconsin. All non-motorized sailboats 12’ and greater must also be registered. Voluntary registration of non-motorized canoes or boats is also available. Any boat from another state or country must be registered in Wisconsin if it is used in this state for more than 60 consecutive days.  
Boat Shelter Permit - Waterway & Wetland Permit
A permit is required to construct a permanent boat shelter in a lake or river. Applies to any person wanting to construct a permanent boat shelter.A boat shelter is a structure designed and constructed for the purpose of providing cover for a place where a boat is moored. A boat shelter must be attached to a pier, and may have a roof but may not have walls or sides. Seasonal boat shelters may be placed without a permit if they meet certain regulations. Permanent boat shelters require a permit, and are not permitted on lakes or flowages having less than 500 acres of surface water or on most rivers, streams or creeks.

Boats documented with the US Coast Guard must be registered in Wisconsin in addition to being documented.  
Boat, Fleet Registration
Fleet registration offers reduced registration fees for boats. A person owning or holding 3 or more boats may, at the person's option, pay a reduced rate for these boats instead of the fees which otherwise would be payable. The fee for the issuance or renewal of certifictes of number or registrations for boats under the fleet rate is $27 plus 50% of the fees which would otherwise be applicable for the boats.

Wisconsin has adopted the National Bowhunter Education Foundation's (NBEF) International Bowhunter Education Program, which is acknowledged and accepted worldwide.  
Boxing Club , Amateur
A business entity which conducts amateur boxing and sparring shows.  
Boxing Club, Professional
A business entity engaged in the conduct of professional boxing or sparring shows.  
Boxing Exhibition Permit
Permits are required to conduct amateur and professional shows. A licensed amateur boxing club must obtain a boxing show permit prior to conducting an amateur exhibition. A licensed professional boxing club must obtain a boxing show permit prior to conducting a professional show. A permit is required for each show or exhibition that is to be held.  
Boxing, Professional
License required for a person to perform as a professional boxer in a professional show for compensation. A professional show means any boxing or sparring event the admission to which requires the purchase of a ticket or the payment of anything of value. To participate in a professional show a boxer must hold a current professional boxer license issued by the department under ch. RL 115, Wis Admin. Code and a current federal identification card. The show must be approved under s. RL 110.04, Wis Admin. Code.

Bridge Permit - Waterway & Wetland Permit
A permit is required to construct a bridge crossing a waterway. Applies to any person wanting to construct a bridge in or over a waterway, with the exception of state highway projects.  
Broker-Dealer (Firm) License
A broker-dealer is a firm or individual who offers and sells securities to the public. Investment advice given by broker-dealers is usually incidental to the completion of purchase and sale transactions in securities. Compensation is normally in the form of commission but some firms now charge one fee for all services, based on the value of assets in the account. All broker-dealers, regardless of their location, must be licensed to conduct business in Wisconsin unless they qualify for an exemption from licensing. There is no de minimis or sophisticated investor exemption for broker-dealers.  
Building Contractor Registration
No person or entity may engage in a construction business or offer to engage in a construction business that affects public buildings, places of employment, one- and 2- family dwellings, public swimming pools and water attractions, unless the person or entity holds a registration issued by the department as a registered building contractor.

Bulkhead Line Approval - Waterway & Wetland Permit
A permit is required to establish a bulkhead line along the shore of any navigable waters. A bulkhead line is a legally established shoreline, which may be adopted by a municipal ordinance and must be approved by DNR. The main purpose of a bulkhead line is to regularize the shoreline, and a secondary purpose is to establish a recoverable shoreline. Only municipalities are eligible to apply for a bulkhead line approval.  
Burning Permit Form 9400-196
This temporary burning permit grants the individual holder the right to burn small amounts of dry leaves, plant clippings, brush, paper, cardboard and clean untreated-unpainted wood as long as weather conditions do not pose a fire hazard. Note: small businesses, commercial enterprises, and industries may not use burn barrels or engage in other kinds of open burning for any waste generated by the businesses.

Cancer Drug Repository
A program that allows individuals to donate used prescription medications to a pharmacy or medical facility.  The donated medications will be used by people who need them but have no other means to obtain the medications.  
Captive Wild Animal Farm License
A captive wild animal license authorizes the holder to possess, propagate, kill, exhibit, purchase and sell live captive wild animals of the species specified by the department on the license.  
Cemetery Authority
A business entity engaged in the operation of a cemetery in the state of Wisconsin. For the purposes of Department registration, cemetery authority means a cemetery association created under s. 157.062, Wis. Stats., or a corporation organized under Ch. 180 or 181, Wis. Stats., which operates a cemetery.  
Cemetery Merchandise Warehouse
A business entity used as a place of storage for cemetery merchandise sold under a preneed sales contract. Cemetery merchandise refers to goods associated with the burial of human remains, including monuments, markers, nameplates, vases and urns, and any services that are associated with supplying or delivering those goods or with the burial of human remains and that may be lawfully provided by a cemetery authority. The term does not include caskets or outer burial containers. No person may own or operate a cemetery warehouse unless the warehouse is approved. The purpose of the approval process of cemetery warehouses is to assure that they have sufficient bonding, so that cemetery merchandise stored until delivery will be available when delivery is expected.

Certification available to any individual, business, or unit of government that conducts an environmental investigation and cleanup of a contaminated property, following state requirements with Department of Natural Resources staff oversight. This certification provides the entity with exemption from future environmental liability.  
Certificate of Status
Certificate under hand and seal of the Department of Financial Institutions, stating the entity (corporation, limited liability company, etc.) is of record with the department; that the entity has, within its most recently completed report year, filed the required report; and that the entity has not filed articles of dissolution.  
Certificate of Status - State Chartered Bank, Savings Bank, or Savings & Loan

Change of Ownership or Control

A business entity that is described in section 501(c)(3) or the internal revenue code and that is exempt from taxation under section 501(a) of the internal revenue code, or a person who purports to be established for a charitable purpose. A license to operate a family day care center for 4 to 8 children.

Child Labor Work Permit
A child labor permit is required for an employer to hire someone under the age of 18 to work in any job with the exception of agriculture or domestic service work. Employers must have a work permit on file for each minor employed, before the minor begins to work. A work permit is only valid for the employer to whom it is issued and for the work discribed on the permit. Work permits are issued at various locations throughout the state. In many communities, work permits may be obtained at the local high school. The permit will not be issued if the work is prohibited by law.  
Child Placing Agency
A child welfare agency licensed to place children in foster homes, group homes and adoptive placements and other child welfare services.  
Chiropractor
License required for a person to perform or examine into the fact, condition or cause of departure from complete health and proper condition of the human; to treat without the use of drugs as defined in s. 450.01(10), Wis. Clean Fuel Fleet Program Annual Compliance Plan Report
Organizations in the Covered Area (refer to Prerequisites section) are required to purchase a specific percent of low emission vehicles as part of the state’s effort to reduce vehicle emissions. Those organizations are required to use this form to list the number of purchases of 1999 or newer Covered Fleet Vehicles.  
Clean Fuel Fleet Program Fleet Registration Form
This form is required of organizations with Covered Fleets in the affected counties (refer to Prerequisites section) to list the vehicles in their fleets in order to verify they are purchasing the required percent of low emission vehicles in the state’s effort to reduce vehicle emissions.

A Community Currency Exchange is a person engaged in the business of and providing facilities for cashing checks, drafts, money orders and all other evidences of money acceptable to such community currency exchange for a fee, service charge or other considerations. Certain persons are exempt from s.218.05, Wis Stats. The exemptions and a complete definition are set forth in Wisconsin Statutes 218.05(1)(b)  
Community Mental Health Center
Certification in the Medicare program to provide partial hospitalization services.  
Construction Inspector, Uniform Dwelling Code (UDC) One- and Two-Family Home
Allows a person to inspect one- and two-family dwellings for the purpose of administering and enforcing chs. Comm 20 to 22.

No person may operate a school unless the school holds a current license. School means any facility, other than a specialty school, that offers instruction in barbering or cosmetology, aesthetics electrology or manicuring. Specialty school means an establishment that offers instruction to no more than one person at a time and does not charge tuition for the training.  
Elevator Apprentice

Registration required for a person to work under the general supervision of a licensed Elevator Mechanic through the Apprenticeship Bureau for the purpose of being trained and educated to erect, construct, alter, replace, maintain, repair, remove or dismantle conveyances.

Emergency Permit or Special License - Teaching or Pupil Services

All Wisconsin public school education professionals are required to have an appropriate teaching, pupil services or administrative license, which reflects the grade levels/subjects for which they were hired. However, in cases where a CESA or school district is unable to find a fully qualified, licensed individual to fill a teaching or pupil services position an emergency permit or license may be requested by the CESA or district administrator.

Applicants who are licensed (or eligible for licensure) in Wisconsin but are needed for an assignment outside their area of licensure may be eligible for an emergency license.

Applicants who did not complete an educator preparation program but have a bachelor's degree from an accredited institution may be eligible for an emergency permit.

Enclosure Permit - Waterway & Wetland Permit
A permit is required to enclose a portion of a navigable waterway. A DNR permit is required to direct, place, or restrict navigable waters into an enclosed drain, conduit, storm sewer or similar structure. Only municipalities are eligible to apply for an enclosure permit. DNR may only grant the permit if it finds that the project is in the public interest, will not violate public rights, and will not endanger life, health or property.

Endangered Species Regulatory Review
According to Section 29.604, Wis. Stats., it is illegal to take, transport, possess, process or sell any wild animal that is included on the Wisconsin Endangered and Threatened Species List. In addition, it is illegal to remove, transport, carry away, cut root up, sever, injure or destroy a wild plant on the Wisconsin Endangered and Threatened Species List on public lands. Forestry, agricultural and utility practices are exempted from the taking prohibitions of listed plant species. In order to find out whether or not there is potential for a proposed project to impact rare species, an applicant can request an Endangered Resources Review be conducted. This Review provides a letter back to the applicant explaining what rare species, natural communities, or natural features tracked in the Natural Heritage Inventory database are found in or near the proposed project area. The letter will also explain whether additional steps should be followed to assure compliance with the Wisconsin Endangered Species Law. This letter may be necessary for other Department of Natural Resources permits.

Retail Class "B" (beer) or "Class B" (intoxicating liquor) permits issued by the Department of Revenue to clubs that are operated solely for the playing of golf or tennis (commonly known as country clubs) and to clubs that are operated solely for curling, ski jumping or yachting.

The department issues these permits providing the club is not open to the general public and the local municipality cannot issue the permit needed. The permit issued authorizes the sale of the related alcohol beverage for consumption on the premises where sold.

Any person, sole proprietorship, partnership, limited liability company, corporation, association, the State of Wisconsin or any political subdivision, but not the United States or any of its agencies must obtain a general aviation fuel tax license if you deliver aviation fuel into either the supply tank of an aircraft or the bulk storage tank of someone who will put the aviation fuel into the supply tank of an aircraft. Persons who want a fuel license must hold a Business Tax Registration (BTR) Certificate. General avaiation fuel is all combustible gases and liquids placed in the supply tank of an aircraft suitable for generating power to propel the aircraft. Examples of aviation fuel are products commonly or commercially known as aviation gasoline (avgas), jet turbine fuel (kerosene), and unleaded gasoline (mogas).

Any person who transports motor vehicle fuel, general aviation fuel or alternate fuels on highways in Wisconsin must be registered with the Department. The Department will issue a transporter number which must be displayed on all vehicles/trailers owned by such persons which will transport.

Any person, sole proprietorship, partnership, limited liability company, corporation, association, the State of Wisconsin or any political subdivision, but not the United States or any of its agencies must obtain a motor vehicle fuel tax license if they want to act as a supplier or "restricted supplier" of motor vehicle fuel in Wisconsin. "Suppliers" include persons who: Import, or acquire immediately upon import, gasoline or diesel fuel by pipeline or marine vessel from a state, territory or possession of the United States or from a foreign country into a terminal. Produce, manufacture, or refine gasoline or diesel fuel within Wisconsin. Acquire motor vehicle fuel pursuant to an industry terminal exchange agreement. Produce alcohol or alcohol derivative substances in Wisconsin, import alcohol or alcohol derivative substances into a terminal or bulk plant, or acquire them immediately upon import by truck, railcar, or barge into a terminal. The above suppliers must also be registered with the federal government under 26 USC 4101 for tax-free transactions in gasoline and diesel fuel. Suppliers also include "restricted suppliers" who may only: Import motor vehicle fuel and/or other petroleum products into Wisconsin from an out-of-state bulk plant, providing the transporting vehicle is not capable of carrying more than 4,200 gallons and the delivery location is no more than 25 miles inside the Wisconsin border. Remove motor vehicle fuel from a bulk plant in Wisconsin in a vehicle capable of not carrying more than 4,200 gallons to a destination not more than 25 miles outside the Wisconsin border.

Certified entity that performs, supervises, or offers to perform or supervise regulated asbestos abatement activities limited to non-friable asbestos-containing material that remains non-friable, on the exterior, or building envelope of a facility.  
Exterior Asbestos Supervisor
A person trained and certified to conduct or oversee exterior asbestos projects that disturb non-friable asbestos-containing materials on building exteriors, and has the authority to require changes in performance practicesor to halt the project, and who may collect bulk samples to ascertain the presence of asbestos in non-friable materials.  
Exterior Asbestos Worker
A person trained and certified to work with non-friable asbestos containing material (ACM) on building exteriors under direct supervision of a certified supervisior.

A fertilizer labeled solely for organic crop production is exempt from the permit requirements if all of the following apply:

1.  The fertilizer qualifies, or all of its ingredients qualify, under 7 CFR 205 for use in organic crop production.

2.  The fertilizer label discloses the listing or approval under subd. 1.

3.  The fertilizer label conspicuously states that "This product is intended for use according to an approved organic system plan."

4.  The manufacturer or distributor makes no performance claims for the product.

Fire Sprinkler Contractor, Automatic
License required for a person to design, install, inspect, test, and maintain automatic fire sprinkler systems. Every company engaging in the business of installing fire sprinkler systems must have a licensed automatic fire sprinkler contractor, who is the responsible supervising credentialed professional.  
Fire Sprinkler Contractor, Automatic - Maintenance Only
Registration and certification required for individuals to work only within the facilities or properties of the employer to modify or extend an existing automatic fire-sprinkler system by no more than 15 sprinklers per project and only within the facilities or properties of the business establishment where the person is employed.

Registration and certification required for a person to maintain automatic fire sprinkler systems under the general supervision of a registered automatic fire sprinkler contractor-maintenance, and to perform this work only within the facilities or properties of the business establishment where the person is employed.

License required for a person to install automatic fire sprinkler systems under the general supervision of an automatic fire sprinkler contractor.

Firearms Permit

An agency shall apply to the Department for a permit to authorize any of its owners or employees to carry a firearm, and all persons so authorized must first have a certificate of proficiency.

First Aid and CPR Training For Employes of Fitness Centers
Approval of basic first aid and basic CPR training curriculum for fitness centers ONLY when a fitness center does not use American Heart Association or American Red Cross curriculum to train employees.

Certification required for a person to use an advanced skills in a prehospital setting.  
Fish (Private) Management Permit
Any person or persons owning all of the land bordering a navigable lake that is completely landlocked (no inlet or outlet streams) may apply for a permit to remove, destroy or introduce fish in such lake. A permit may only be granted if the DNR determines after a public hearing that it is not contrary to public interest or law. All work done as a result of the permit needs to be supervised by the DNR and expenses of the DNR supervision paid by the permitee. This permit does not apply to navigable lakes that are freeze-out ponds used as fish farms.  
Fish Crib, Standard - Waterway & Wetland Permit
A permit is required for placement of a fish crib on the bed of a navigable waterway. This applies to anyone wanting to place a fish crib or other type of habitat protection structure on the bed of a navigable waterway.

Required for anyone who buys, barters, sells or solicits fish in any manner for sale to anyone other than a consumer.  
Fish Farm Registration
Registration required for facilities to hatch fish or hold live fish for sale or distribution, introduction into state waters, fishing, use as bait or fertilizer, use as human food or animal feed, education, demonstration, or research; or to hold live fish or fish eggs owned by another person.  
Fish Import Permit
Permit required to import native live fish or fish eggs into Wisconsin.  
Fish Import Permit (non native fish)
Permit required for a person to import any fish, or fish eggs, into Wisconsin of a species not native to Wiscosnin for the purposes of introduction into the waters of the state, use as bait or rearing in a fish farm.  
Fish Stocking Permit
In order introduce, stock, or plant fish in any water of the state, and individual or group must obtain a stocking permit from the Department of Natural Resources (DNR). The fish must be certified by a qualified health inspector to meet health standards.

A Fishing license is required for both residents and nonresidents and allows fishing in any waters of the state. A Great Lakes Salmon/Trout stamp is required to fish salmon or trout in outlying waters in addition to a fishing license. Forest Products Permit – Form 2460-8
Lands administered by the Department offer an opportunity to citizens seeking forest products. Permits are attainable for small amounts of firewood, Christmas trees, boughs or other forest products so long as the products are surplus to the property’s needs and objectives. The property manager or superintendent administers the permits and the Forester servicing the property is responsible for appraising the forest products and assuring the removal or manner of cutting is not detrimental to the property objectives. These permits are for small amounts of products not to exceed $500 in appraised value. Up to 10 cords of firewood and 5 Christmas trees are permitted. The duration of the permit can not exceed 1 year. The entire payment is required in advance of any harvest. Permits may be restricted in special management zones or when inconsistent with the management of the property

Residents having an established post or place of business in Wisconsin where they carry on the business of buying, bartering, trading and otherwise obtaining raw or dressed furs. Nonresidents buying, bartering trading or otherwise obtaining raw furs from trappers or from fur buyers or fur dealers for purposes of resale.

Registration required for a person to perform the practice of professional geology which is the performance of or the offer to perform any geologic service or work in which the public welfare or the safeguarding of life, health, environment or property is involved.

Incidental Take Permit
According to Section 29.604, Wis. Stats., it is illegal to take, transport, possess, process or sell any wild animal that is included on the Wisconsin Endangered and Threatened Species List. In addition, it is illegal to remove, transport, carry away, cut root up, sever, injure or destroy a wild plant on the Wisconsin Endangered and Threatened Species List on public lands. Forestry, agricultural and utility practices are exempted from the taking prohibitions of listed plant species. The Wisconsin Endangered Species law allows for the taking of a protected species if the taking is not for the purpose of, but will be only incidental to, the carrying out of an otherwise lawful activity. Authorization for Incidental Take can occur through an incidental take permit if there is no state involvement in the project; or through the Incidental Take Consultation process if the state conducts, funds, or approves any or all of the activity. Authorization is determined by the DNR based on the following criteria: 1. Any adverse effects to the protected species must be minimized to the maximum extent practicable, 2. The activity will not jeopardize the continued statewide existence and recovery of the species or the whole plant-animal community of which it is part, or habitat that is critical to the species existence within the state. 3. Activity must benefit the public health, safety or welfare that justifies the activity. In addition, the department must provide public notice of the proposed activity for at least 30 days. If the public provides additional information that would impact the authorization decision, it will be considered prior to final authorization.

Lead Training Course Accreditation
Accreditation of a training course for a specific job classification on the basis of its compliance with Subchapter III of Chapter DHS 163, WI Administrative Code.  
Lead-Safe Company

Certified entity that performs, advertises, claims to provide or offer to perform renovation activities in pre-1978 housing or child occupied facilities.

A business entity engaged in the practice of manicuring. Manicuring means cleansing, cutting, shaping, beautifying or massaging limited to the hands, feet or nails of the human body. No person may practice manicuring in an establishment unless the establishment is licensed.  
Manicuring Instructor
License required for a person to perform practical instruction in manicuring in a school licensed by the Department  
Manicuring School
A business entity engaged in the practice of teaching manicuring. Manicuring means cleansing, cutting, shaping, beautifying or massaging limited to the hands, feet or nails of the human body. No person may operate a school unless the school holds a current license. School means any facility, other than a specialty school, that offers instruction in barbering or cosmetology, aesthetics electrology or manicuring. Specialty school means an establishment that offers instruction to no more than one person at a time and does not charge tuition for the training.  
Manicurist
License required for a person to perform manicuring for compensation. Manicuring means cleansing, cutting, shaping, beautifying or massaging limited to the hands, feet or nails of the human body.  
Manufactured Home Certificate (Title)
Required to transfer ownership of a manufactured home (mobile home) after selling it from one party to another party.  
Manufactured Home Community License
Allows an owner to operate a community for three or more manufactured homes used for dwelling or sleeping purposes.

License required for a person to perform psychotherapeutic and marital or family systems theories and techniques in the assessment, marital or family diagnosis, prevention, treatment or resolution of a cognitive, affective, behavioral, nervous or mental disorder of an individual, couple or family.

Meat Broker or Distributor Registration
A meat broker or distributor must hold an annual registration certificate from the department under s. 97.42 (2)(d), Stats

License required for plants and premises to process meat or poultry that will be resold for people to eat. Not required of plants inspected by the USDA.  
Meat/Poultry, Custom Slaughtering or Processing License

Day treatment is a basic element of the mental health program providing treatment while the patient is living in the community. Day treatment provides treatment services for patients with mental or emotional disturbances, who spend only part of the 24 hour period in the services. Day treament may be conducted during day or evening hours.  
Mental Health Day Treatment Services for Children
Mental health day treatment services for children are non-residential care provided on prescription of a physician in accordance an individually prepared plan of treatment that is based upon a multi-disciplinary assessment designed to alleviate emotional or behavioral problems.

Inpatient mental health services means medically oriented treatment, psychotherapy and other mental health services provided in a hospital or residential setting on a 24 hour per day basis to enable a person with a mental disorder or a mental disorder in combination with other impairments to function successfully without 24 hour per day services.

Emergency mental health services means a coordinated system of mental health services which provides an immediate response to assist a person experiencing a mental health crisis

Metal Detector User Permit
Use of a metal detector on Department of Natural Resources property is authorized only if the property superintendent issues a permit.

The purposes are to prevent interference with the enjoyment of recreational facilities by the public, to reduce the potential for damage to the resources, and reduce the potential for loss of historical or archaeologically significant items.

 
Metallic Mineral Exploration License
License required to conduct metallic mineral exploration drilling.  
Metallic Mineral Mining Permit
Permit required to conduct metallic mining operations in Wisconsin. Metallic mining operations are to be conducted in an environmentally acceptable manner in compliance with all applicable statutes and rules. The lands disturbed by the operation must either be converted to a specified alternate use or fully reclaimed. Operation must be completed in conformance with the approved Mining Plan and Reclamation Plan.  
Metallic Mineral Prospecting Permit
Permit required to conduct metallic mineral prospecting (bulk sampling).  
Metallic Mining Waste Facility License
License required to store or dispose of metallic mining wastes. Issued following site construction and certification that the construction was in compliance with approved plans.  
Migrant Labor Camp Certificate
Certificate that permits migrant employers to operate a migrant labor camp. Location of the migrant labor camp, date of issuance and expiration date are listed.  
Migrant Labor Contractor I.D. Card
Permit that allows a labor contractor to recruit migrant workers into this state. Includes the labor contractor name, year permit issued, and signature of the Director of the Bureau of Migrant and Refugee Services.  
Milk & Cream Tester License
License required for a person to perform dairy quality tests that ascertain fat content and determine payment levels.

Mine Shaft Excavation

Before an excavation of a shaft may be commenced, a permit shall be obtained rom the Department of Commerce. This does not apply to shafts less than 50 feet in depth wherein persons are not employed, shafts not equipped with power driven hoists used for hoisting persons in and out of shafts, or shafts not covered with a flammable building.

 
Minimum Wage, Special
License required for employers wishing to pay a physically or mentally challenged worker or a student learner less than minimum wage. This license is required to hire workers whose disabilities impair their ability to perform their work, under a special minimum wage that is less than that otherwise prescribed. The rate must reflect the productivity of the worker compared to the productivity of a worker not disabled for similar work, and to the wages paid to experienced workers performing the same or similar work in the vicinity.  
Minority Business Enterprise Certification
The certification process determines if a business is a minority business for the purpose of participating in the state's minority business program under s. 16.75(3m) or other statutory minority business procurement program.  
Miscellaneous Structure Permit - Waterway & Wetland Permit
A permit is required for the placement of any structure in a waterway. Specific application forms and information are available for certain structures, including boat ramps, boat shelters, bridges, buoys, culverts, dry hydrants, fish cribs, piers, pea gravel blankets, pilings, seawalls, and shore protection structures. For all other projects where a specific application form is not available separately, a miscellaneous structure permit is required. Placement of fill on the bed of a waterway is generally prohibited. Applies to anyone who wants to place a structure or material into a waterway.  
Mobile Air Conditioning Registration
Registration required for businesses to sell or install refrigerant for mobile air conditioners, or to perform repairs that may result in its release.  
Moped Dealer
License required for a business to sell mopeds only.  
Mortgage Banker
A corporation, partnership, limited liability company (LLC) or individual who originates loans for another person or for itself as payee on the note, sells loans or interests in loans to another person, services loans or land contracts, or provides escrow services.  
Mortgage Broker
A corporation, partnership, limited liability company (LLC) or individual who, on behalf of a loan applicant or an investor and for commission, money or other thing of value, finds a loan or negotiates a land contract, loan or commitment for a loan or engages in table funding.  
Motel
A permit to operate a place that furnishes on-premise parking for motor vehicles of guests as part of the room charge, without extra cost, and that is identified as a "motel" rather than a "hotel" at the request of the operator.  
Motor Carrier Authority
Intrastate operating authority is required to haul a commodity or product for someone else. This is also known as being a "for hire" carrier. If you are a Wisconsin based carrier or an interstate "for hire" carrier and wish to haul for hire on an intrastate basis, it is necessary to obtain operating authority from the Wisconsin Department of Transportation.  
Motor Club Services
Certificate of authority required for a company to offer for sale any motor club service to members, such as emergency road service, towing, bail bonds, map and touring service, etc.  
Motor Vehicle Auction Dealer
License required for a business to hold wholesale auctions in which it sells vehicles owned by dealers to other dealers.

A motor vehicle Buyer's license allows employees or those who contract with licensed motor vehicle dealers or wholesalers to buy motor vehicles at motor vehicle wholesale auctions.

License required for a business to manufacture or assemble motor vehicles or to make major manufacturing alterations to vehicles.

Music Therapist
License required for a person to perform the professional, therapeutic use of music in the service to individuals with needs in mental health, physical health, habilitation, rehabilitation, special education, self-care or personal growth. The purpose of music therapy is to assist and empower individuals to attain or maintain their maximum level of functioning and highest quality of life. "Music therapy" may include the use of elements or other art forms.  
Natural Waterbody Permit for Fish Farms
A permit is required to use natural water bodies as a fish farm or part of a fish farm. Fish farms that were licensed by DNR as of January 1, 1998 and use a natural water body may be grandfathered. The use of natural water bodies after January 1, 1998 for new fish farms is limited to freeze-out ponds. Applies to anyone who wants to use a natural water body as a fish farm.  
Nesting Structure Permit - Waterway & Wetland Permit
A permit is required for the placement of a bird nesting platform, a wood duck house or similar structure on the bed of a navigable water, unless the project meets certain standards where only notification is required. A riparian owner may place and maintain a nesting structure on the bed of navigable waters for the purpose of improving wildlife habitat without a permit, if the nesting structure complies with the standards found in NR323, Wisconsin Administrative Code and if the riparian owner submits notification to DNR. If the project will not comply with code standards, a DNR Miscellaneous Structure permit is required.  
Nondomestic Insurers, NAIC Uniform Certificate of Authority Application (UCAA)
Certificate of authority required for a nondomestic insurer to do insurance business in the state. May be a mutual company or a stock company.  
Nonmetallic Mining Permit - Waterway & Wetland Permit
A permit is required to dredge materials from the bed of any navigable waterbody, create a pond within 500 feet of a navigable waterway, or grade in excess of 10,000 square feet on the bank of a navigable waterway. Additional requirements apply when the purpose of the project is to extract nonmetallic material for resale or personal use. Applies to anyone who wants to extract nonmetallic material for resale or personal use, where the extraction requires a permit for dredging, pond construction or grading in or near a navigable waterway.

Registration certificate required for a person to perform the planning, organizing, directing and controlling the operation of a nursing home, or who in fact performs such functions, whether or not such functions are shared by one or more other persons.  
Occupational Therapist
License required for a person to perform purposeful activity with persons who are limited by physical injury or illness, psychosocial dysfunction, developmental or learning disability or the aging process, in order to maximize independent function, prevent further disability and achieve and maintain health and productivity, and encompasses evaluation, treatment and consultation services that are provided to a person or group of persons.  
Occupational Therapist/Physical Therapist, Public School (includes therapy assistants)
License required of individuals employed by Wisconsin public schools to provide occupational/physical therapy or to assist occupational/physical therapists in delivering treatment as required by students' individual education plans per Chapter PI 11.  
Occupational Therapy Assistant
License required for a person to perform or assist in the practice of occupational therapy under the supervision of an occupational therapist.  
Oil and Gas Exploration License
License required to conduct oil and gas exploration drilling.  
Optometrist
License required for a person to perform any optometric means, including topical ocular diagnostic pharmaceutical agents to determine the visual efficiency of the human visual system.  
Optometrist - DPA & TPA Certificates
License required for a person to perform any optometric means, including topical ocular diagnostic pharmaceutical agents to determine the visual efficiency of the human visual system.  
Pea Gravel - Waterway & Wetland Permit
A permit is required to place a layer of pea gravel on a navigable waterway. Applies to anyone who wants to place pea gravel or other material on the bed of a navigable waterway.  
Peddler
License required for a person to perform as one who travels about selling wares. The peddler's license applies to any ex-soldier of the United States in any war, who has a 25% or greater disability or has a cardiac disability recognized by the U.S. Veterans Administration, and any person disabled to the extent of the loss of one arm or one leg or more or who has been declared legally blind.  
Perfusionist
The professional who operates the heart-lung machine during surgery is the perfusionist.

"Perfusion" means that branch or system of treating the sick that is limited to the operation and management of extracorporeal circulation to support, temporarily replace, measure, treat, or supplement the cardiopulmonary and circulatory system of a patient, including, when necessary to and part of the management and operation of extracorporeal circulation, the use of blood testing and advanced life support techniques and technologies, autotransfusion, and the administration of blood, blood products, and anesthetic and pharmacological agents.

A person holding a license to practice perfusion may practice perfusion under the orders and supervision of a physician.

 
Permit for Use of Non-Standard Minnow Gear on Inland Waters
A person holding a current bait dealer's license may apply for a permit for the use of special gear not authorized in the existing regulations.  
Personal Care Agency - Freestanding
Initial approval of a personal care agency to become freestanding and eligible to seek approval for Medicaid reimbursement. Approval follows the determination that the agency has met fit and qualified standards.  
Pesticide Emergency Use Permit
Permit authorizes use of a prohibited pesticide (except DDT) if use is necessary in an emergency.

License required for a person to engage in the practice of pharmacy, granted to individuals who meet education, training, and examination requirements.  
Pharmacy
A business entity engaged in the dispensing of drugs. To dispense means to deliver a prescribed drug or device to an ultimate user or research subject by or pursuant to the prescription order of a practitioner, including the compounding, packaging or labeling necessary to prepare the prescribed drug or device for delivery.  
Physical Therapist
License required for a person who has graduated from a school of physical therapy to perform the treating of the sick which is limited to therapeutic exercises with or without assistive devices, and physical measures including heat and cold, air, water, light, sound, electricity and massage; and physical testing and evalutaion. The use of roentgen rays and radium for any purpose, and the use of electricity for surgical purposes including cauterization, are not part of physical therapy.  
Physical Therapist Assistant

A physical therapist assistant is an individual who holds a license as a physical therapist assistant granted by the affiliated credentialing board.

 
Physician (License to Practice Medicine and Surgery)
License required for an individual to practice medicine and surgery. Applicants must possess the degree of doctor of medicine or doctor of osteopathy.  
Physician Assistant
Certificate required for a person to perform patient services under the supervision and direction of a licensed physician.  
Picnic Shelter Reservation
A reservation grants a group exclusive use of a shelter, picnic area, auditorium, or amphitheater for a picnic or other event.  
Pier - Waterway & Wetland Permit
A pier may be placed in a navigable waterway by the waterfront property owner provided certain conditions are met. A permit is required for anyone who wants to place a pier(s) that exceeds these conditions. For example, to place a long pier or dock a large number of boats a pier permit would be required. A permit is required for all commercial or public marinas.  
Pierhead Line Approval - Waterway & Wetland Permit
A permit is required to establish a pierhead line along the shore of any navigable waters. A pierhead line is a legally established distance from shore, which may be adopted by a municipal ordinance and must be approved by DNR. The main purpose of a pierhead line is to establish the maximum length for piers, and must be in the interest of the preservation and protection of a municipal harbor or of public rights in navigable waters. Only municipalities are eligible to apply for a pierhead line approval.  
Piling - Waterway & Wetland Permit
A permit is required for placement of pilings (typically a cluster of piling lashed together). Pilings or cluster piles are used on some waterways to protect in-water structures from severe ice damage. A permit is required for anyone who wants to place a piling or cluster of pilings on the bed of a navigable waterway.  
Pipelayer
This registration allows a person to install or modify water services, private water mains, sanitary building sewers, storm building sewers, or private interceptor main sewers under the general supervision of a licensed utility contractor, licensed master plumber, or a licensed master plumber-restricted service.  
Plan Review, Anhydrous Ammonia (NH3) Systems
Plans are submitted to Department of Commerce staff for review and approval by contractor who designs, installs or assembles Anhydrous Ammonia Systems.  
Plan Review, Commercial Buildings
Written approval of building plans required to get a permit for new construction of, addition to, or alterations of buildings containing 25,000 cu ft total volume or more.  
Plan Review, Compressed Natural Gas (CNG) Systems
Plan review is submitted by contractor who designs, installs or assembles Compressed Natural Gas Systems.  
Plan Review, Elevator
Approval of plan required for installation of elevators, escalators, or lifts in new, additions to, and alterations of buildings with a total volume of 25,000 cu ft or greater.

For the installation of fire alarm and/or suppression systems in certain occupancies of buildings with a total volume of 25,000 cu ft or greater.

Plan Review, Liquid Natural Gas (LNG) System

Plan review and approval are required to create or expand a manufactured home community (mobile home park).  
Plan Review, Passenger Ropeways (Tramways, Lifts and Tows)
Plan for design, installation, or assembly of passenger ropeways need to be submitted and approved by Depart of Commerce staff. Also applies to all alterations of and additions which affect a system's design, structural strength or operation, or which replaces any piece of major equipment on a system.

Allows a person to conduct plumbing inspections for one- and two-family dwellings for the purpose of administering and enforcing chs. Comm 25, 82 and 84.  
Plumbing Learner - Restricted Appliance
Allows a person to install and modify water heaters, water softeners, water treatment devices and other items in connection with an existing water supply system which do not require direct connection to the drain system.

Allows a person to install or modify privately owned wastewater treatment systems and components.  
Podiatrist
License required for a person to perform podiatry or podiatric medicine and surgery granted by the board.  
Pond Permit - Waterway & Wetland Permit
A pond may be created in many ways, and a permit is required when a pond is created near or connected to a waterway, or within a wetland. A permit is required to create a pond by 1) constructing, dredging or enlarging any artificial pond or waterway within 500 feet of a waterway, 2) constructing a watercourse that will be ultimately connected to a waterway, 3) enlarging or connecting any natural or artificially constructed pond or waterway with any existing waterway, 4) constructing any pond or waterway project that will directly or indirectly affect wetlands, or 5) constructing a dam on a waterway. Applies to anyone who wants to create a pond near or connected to a waterway, or within a wetland.  
Portable Asphalt Plant Relocation
Portable asphalt plants must provide DNR with notification of intent to relocate 20 days prior to relocation.  
Power Piping/Welded Refrigeration Piping Installation Registration
Registration application is submitted by contractors to obtain inspection and approval before fabrication of piping system begins.  
Prescription Drug or Device Distributor
A business entity engaged in the practice of wholesale distribution of prescription drugs or devices. It includes, but is not limited to, manufacturers; repackers; own-label distributors; private-label distributors; jobbers; brokers; warehouses, including manufacturers' and distributors' warehouses, chain drug warehouses, and wholesale drug warehouses; independent wholesale drug traders; and pharmacies that conduct wholesale distributions not coincident to the compounding, packaging, labeling and dispensing of prescription drugs and devices  
Pressure/Vacuum Certification, Gasoline Delivery Transports
Annual pressure/vacuum certification sticker for gasoline delivery vessels, which allows the vehicle to obtain gasoline from bulk gasoline terminals.  
Private Detective
License required for a person to perform as a private detective as follows: 1. A person who acts as, uses the title or otherwise represents that the person is a private detective, private investigator or special investigator. 2. A person engaged for compensation or other consideration on behalf of another, in investigating or otherwise obtaining or furnishing information relating to any of the following: a) crimes or wrongs done or threatened against The United States, any state or, territory, or any political subdivision thereof. b) the identity, conduct, business, honesty, activity, movement, whereabouts, affiliations, associations, transactions, acts, reputation or character of any person, if such information is obtained in secret, without the knowledge of the subject c) the location, disposition or recovery of lost or stolen property d) the cause or responsibility for fires, libels, losses, accidents, damage, injury or death e) securing evidence to be used before any court, board, officer, or investigating committee 3) A person who acts as a private security person and does not wear a uniform, including one who provides personal protection of individuals from bodily harm or death.  
Private Detective Agency
A business entity engaged in the practice of private investigation or the provision of security guard services. Private detective agency means an individual, limited liability company, partnership, or corporation holding a private detective agency license issued by the Department of Regulation and Licensing. An agency license allows a private detective agency to employ private detectives licensed by the Department or security guards who have received a permit from a local law enforcement agency.  
Private Employment Agency License
This license is required, for an individual or business, to engage in finding employment for job applicants for a fee.  
Private Employment Agency Registration
Registration is required for an individual or business to engage in helping employers find permanent employees for a placement fee.  
Private Fishing Preserve Permit
This is a license offered to individuals who would like to register a natural, navigable, self-contained body of water as a private fishing preserve with the Department of Natural Resources (DNR). The body of water must have been licensed by the DNR as a private fish hatchery in 1997 in order to be eligible for this permit. This license is appropriate for individuals who would like to register a private fishing preserve. Lake associations, corporations, or other associations formed to establish a private fishing preserve (or any individual representing any of the aforementioned organizations) are not eligible to register for this license.  
Private Onsite Wastewater Treatment System (POWTS) Inspector Certification
Allows a person to inspect privately owned wastewater treatment systems (POWTS) for the purpose of administering and enforcing the provisions of chs. Comm 81 to 87, plumbing code, as an authorized representative of the department or a local governmental unit.  
Private Onsite Wastewater Treatment System (POWTS) Maintainer Registration
Allows a person to evaluate, monitor and maintain private onsite wastewater treatment system (POWTS) components for the purpose of providing the management of a POWTS.  
Private Security Person Permit
Permit required for a person to perform or act as a private security person including any private police, guard or any person who stands watch for security purposes.  
Professional Counselor
License required for a person to perform counseling through various means of applying a combination of human development, rehabilitation and either psychosocial or psychotherapeutic principles, procedures or services that integrate a wellness, pathology and multicultural model of human behavior in order to assist an individual, couple, family, group of individuals, organization, institution or community to achieve mental, emotional, physical, social, moral, educational, spiritual, vocational or career development and adjustment through the life span of the individual, couple, family, group of individuals, organization, institution or community.  
Professional Counselor Certificate
Certification required for a person to perform as a professional counselor in a supervised clinical professional counseling practice or in a position in which the applicant will, in the opinion of the professional counselor section, receive training and supervision equivalent to the training and supervision received in a full-time supervised clinical professional counseling practice. Certification is valid for 24 months for an individual who has received a master's or doctorate degree in professional counseling or its equivalent from a program approved by the professional counselor section and is employed full-time, or has an offer of full-time employment.  
Property Tax, Appraiser
Certification required for persons to perform broad property appraisal and data collection duties under the supervision of a property assessor, as part of a municipal assessing function.

License required for a person to perform psycho-educational evaluation and intervention for the prevention and treatment of educationally relevant problems. Intervention includes service to the student or consultation with persons who affect the student's education program.

Public Librarian Certification
Public library and public library system directors administer public libraries and systems under the direction of boards. Boards must hire directors who qualify for appropriate certification and who maintain that certification through required continuing education. Required for directors of municipal, joint, and county public libraries, and directors of public library systems.  
Public Warehouse Keeper License
License required for business to "store for hire" the property of others.  
Public Water Systems, Capacity Development Certification

Capacity Development is a program that aims to help public water supply systems strengthen their ability to consistently supply safe drinking water to their customers.  The program focuses on assisting system owners and operators, particularly small water systems, with improving their technical abilities, managerial skills and financial resources to comply with the Safe Drinking Water Act requirements. 

Certification is required of public water systems to comply with the Safe Drinking Water Act requirements on the basis of their technical, managerial, and financial resources. Applicable systems include new municipal; other-than-municipal (OTM) such as apartment complexes and mobile home parks; and nontransient, noncommunity public water systems such as schools, industries, and businesses.

 
Racing Occupational License
Every individual that participates in the pari-mutuel industry is required to obtain an occupational license. This process includes being fingerprinted and having a background check performed on the individual.

Real Estate Apprentice Salesperson
Apprenticed individual who chooses to receive training in the field of real estate under a licensed broker. The purpose of the apprenticeship is to provide practical experience to persons interested in the field of real estate and who wish to receive proper training under supervision.  
Real Estate Broker
License required for a person to operate a real estate business.  
Real Estate Business Entity
A "business entity" means any organization or enterprise, other than a sole proprietorship, which is operated for profit or that is nonprofit and nongovernmental, including an association, business trust, corporation, joint venture, limited liability company, limited liability partnership, partnership or syndicate. A "business representative" means a director, manager, member, officer, owner or partner of a business entity. The license entitles each business representative of the business entity who is a licensed broker to act as a broker on behalf of the business entity. According to s. 452.12(2)(a), Wis. Stats, a license may be issued to a business entity if the business entity has at least one business representative licensed as a broker.  
Real Estate Salesperson
License required for an individual to engage in the real estate business, employed by a real estate broker.  
Recreational Vehicle Dealer
License required for a business to sell or lease travel trailers (which are less than 45 feet long) to the public.  
Refrigerant Handling Technician Certification
Allows a person to install or service a piece of refrigeration equipment that would release or may release an ozone-depleting refrigerant.  
Refrigeration Equipment, Registration to Salvage or Dismantle
Any private or public entity responsible for salvaging or dismantling any type of equipment (vehicles, appliances, building air conditioning, commercial refrigeration, etc.) that may contain regulated refrigerants (CFCs, HCFCs, HFCs, PFCs, or any blends of these) must register. See DNR Refrigerant Recovery Program.  
Refrigeration Installation Registration, Mechanical
Required for installing mechanical refrigeration equipment.  
Registration Temporary Trip Permit (Nonresident Motor Carrier)

Out-of-state carriers who do not have WI registration or registration through the International Registration Plan (IRP) must purchase a WI temporary trip permit. Trip permits are valid for:

  • Interstate and intrastate operations

  • 72 continuous hours

  • Only one trip into Wisconsin

Reinsurance Intermediary- Firm
License required for a firm to offer reinsurance intermediary services.  
Reinsurance Intermediary-Broker
License may be required for a person to act as a reinsurance intermediary-broker, including a person who places ceded reinsurance in this state and had an office, or does business in this state and has an office outside this state unless it is licensed under a similar law in another state. A person is not required to hold this license if they are included under the license of a firm.  
Reinsurance Intermediary-Manager
License may be required for a person to act as a reinsurance intermediary manager, who has significant authority regarding assumed reinsurance of an insurer including the authority to bind, manage a separate division, department, or underwriting office with responsibility for, or manages all or part of, assumed reinsurance business of a reinsurer, and acts as an agent for the reinsurer. A person is not required to hold this license if they are included under the license of a firm.  
Renderer License (or Grease Processor)
A person who renders dead animal carcasses into grease or other products must be licensed under s. 95.72 Wis. Stats.  
Rental Unit Energy Efficiency Standards, Certificate of Compliance
To provide compliance with energy efficiency standards and satisfy requirements for a new title on a rental property, this document must be recorded with the Register of Deeds.  
Rental Weatherization Inspector Certification
Allows a person to inspect rental units to determine compliance with the weatherization code in ch. Comm 67.  
Representative Permit
An individual employed by or representing a school must have a representative permit before attempting, in places other than the school, to secure enrollment agreements.  
Residential Care Apartment Complex Certification
Independent apartment units, accepting individuals who pay for services themselves or through Medicare payments, in which the following services are provided: room and board, up to 28 hours per week of supportive care, personal care, and nursing services.  
Residential Care Apartment Complex Registration
Independent apartment units in which the following services are provided: room and board, up to 28 hours per week of supportive care, personal care, and nursing services.  
Residential Care Center
A child welfare agency licensed to provide treatment and custodial services for children, youth and young adults. (Formerly know as a Child Caring Institution - CCI)  
Respiratory Care Practitioner
License required for a person to perform respiratory care to include the branch or system of treating the sick which is limited to assisting in the prevention, diagnosis and therapeutic treatment of respiratory disorders by various means, including the administration of medical gases, oxygen therapy, ventilation therapy, artificial airway care, bronchial hygiene therapy, aeorsolization of pharmacological agents, respiratory rehabilitation therapy and other treatment, testing, evalution and rehabilitation procedures performed under the direction of a physician, but not including the use of general anesthetic agents.  
Restaurant
Any building, room or place where meals are prepared, served or sold to transients or the general public, and all places used in connection with the building, room or place and includes any public or private school lunchroom for which food service is provided by contract.  
Retail Food Establishment License
License required for establishments, permanent and mobile, to sell most processed food directly to consumers in a retail setting. Includes grocery stores, convenience stores, mobile units, knockdown stands, and pushcarts. Does not include restaurants.  
Retaining Wall, Sea Wall - Waterway & Wetland Permit
A permit is required for the construction of a seawall or retaining wall along the shoreline of a waterway. Construction of seawalls is only allowed in limited situations, because of their potential for harming fish and wildlife habitat, natural scenic beauty and other public rights in waterways. Erosion control alternatives such as vegetation, or other structural methods such as rock riprap, should be considered. Applies to anyone who wants to place a seawall or retaining wall along the shoreline of a waterway.  
Riprap Shore Protection - Waterway & Wetland Permit
A permit is required for the placement of rock riprap or other structure along the shoreline of a waterway. Many methods and materials are available to protect shorelines from erosion. Rock riprap is one method of shoreline stabilization, and is preferable to solid seawalls or retaining walls that can harm fish and wildlife habitat, natural scenic beauty and other public rights in waterways. Applies to anyone who wants to place rock riprap or other structure along the shoreline of a waterway.  
Rural Health Clinics
A clinic that is located in a rural area with a shortage of health care professionals. Services offered are similar to those received in a doctor's office and are often provided by a nurse practitioner or a physician's assistant.  
Sales & Use Tax, Certificate of Exempt Status
Certificate required to prove exemption from sales and use tax for federal and Wisconsin governmental units and certain nonprofit organizations, including youth organizations, organized and operated exclusively for religious, educational, scientific, or charitable purposes, or for the prevention of cruelty to children or animals.  
Sales & Use Tax, Consumers Use Tax Certificate
Certificate required for a person to regularly store, use, or consume taxable tangible personal property or taxable services in Wisconsin upon which a sales tax was not paid, unless the person is required to have a seller's permit or a use tax certificate. This certificate is used by businesses located in Wisconsin that are not required to hold a seller's permit (i.e., businesses not doing retail sales) who buy products on which sales tax was not paid and the products are used in their business. An example is a consultant doing business in Madison who buys his office supplies from a Chicago-area business that does not collect Wisconsin sales or use tax.  
Sales & Use Tax, Direct Pay Permit
Permit required for a taxpayer to purchase most tangible personal property or taxable services without tax from retailers and to make payments of tax due directly to the Department of Revenue without going through the retailer.  
Sales & Use Tax, Dry Cleaning Facility License

Permit required to operate a dry cleaning facility in this state.  Any person who operates a dry cleaning facility in Wisconsin is required to apply for a dry cleaning facility license.  The license has no expiration date and must be displayed prominently in the facility to which it applies. 

 
Sales & Use Tax, Dry Cleaning Products, Seller
A dry cleaning products fee is imposed on any person who sells a dry cleaning product to a dry cleaning facility.

A "dry cleaning product" is a hazardous substance used to clean apparel or household fabrics. It does not include substances used to launder apparel or household products. 

If you sell a dry cleaning product to a dry cleaning facility, you must register with the Wisconsin Department of Revenue.

 
Sales & Use Tax, Exemption Certificate
This certificate may be used to claim exemption from Wisconsin state, county, baseball and football stadium, and premier resort sales or use taxes. Exemption certificates are signed by purchasers or lessees and are given to sellers or lessors to verify that a transaction is exempt. Sellers and lessors shall exclude from taxable gross receipts transactions for which they have accepted a valid exemption certificate in good faith from a purchaser.  
Sales & Use Tax, Local Exposition Tax Certificate
Retailers making sales in municipalities located wholly or partially in Milwaukee County are required to register and collect local exposition taxes if they (1) sell food or beverages, (2) have a location primarily engaged in the rental of automobiles for periods of 30 days or less, or (3) rent lodging for a continuous period of less than one month.  
Sales & Use Tax, Local Rental Vehicle Tax

If your primary business is the rental or lease of vehicles without drivers for 30 days or less, you are subject to the 5% Local Rental vehicle tax.  The following types of vehicles are covered by this fee: automobiles, station wagons, mobile homes, motor homes, and camping trailers.

Persons who are subject to this tax, must register with the Wisconsin Department of Revenue.

Permit required for individuals, partnerships, corporations, or other organizations to make retail sales or rentals of tangible personal property or retail sales of taxable services in Wisconsin from a Wisconsin location, unless the seller is exempt from taxation. This permit is valid only at the location for which it is issued. If there is more than one business location, the same permit number will be issued with a different letter suffix for each location.  
Sales & Use Tax, State Rental Vehicle Fee (Limousine)

A fee at the rate of 5%  is imposed for the rental of limousines.  If you provide limousine service, you are subject to the this fee and you must register with the Wisconsin Department of Revenue.

A limousine is a passenger automobile which has a capacity of 10 or fewer persons, excluding the driver, with 5 or more seats behind the driver, that is operated for hire, on an hourly basis, and is directed by the person hiring the limousine. It does not include taxicabs, hotel or airport shuttles or buses, buses employed solely in transporting school children or teachers, vehicles owned or operated without charge or remuneration by a business entity for its own purposes, vehicles used in carpools or van pools, public agency vehicles that are not operated as a commercial venture, vehicles operated as part of the employment transit assistance program under sec. 106.26, Wis. Stats., ambulances, or any vehicle that is used exclusively in the business of funeral directing. 

 
Sales & Use Tax, Temporary Event Operator & Seller Information

Operators of a swap meet, flea market, craft fair, consumer trade show, or similar event are required to report certain information to the Wisconsin Department of Revenue. These events are referred to as temporary events. Operators must report the name, address, social security number, and, if available, the Wisconsin tax account number (seller's permit) of each Seller selling merchandise or providing certain taxable services at their temporary event.

Generally, temporary event sellers who have $1,000 or more in sales of tangible personal property and taxable services during the calendar year are required to have a WI seller's permit. 

 
Sales & Use Tax, Use Tax Certificate
Certificate required for every out-of-state retailer engaged in business in this state who is not required to hold a seller's permit, and required for a retailer who is not engaged in business in Wisconsin but elects to collect use tax for the convenience of its Wisconsin customers.  
Sales Finance Company
A sales finance company is a person, firm or corporation engaging in the business, in whole or in part, of acquiring by purchase or by loan on the security thereof, or otherwise, retail installment contracts or consumer leases from retail sellers or lessors in the state. While motor vehicle, motorcycle, recreational vehicle, and manufactured home dealers may also be sales finance companies, they have separate licensing requirements, and should contact the Department of Financial Institutions for more information.  
Salvaged Refrigeration Equipment, Certification for Safe Transport
Any private or public entity responsible for transporting appliances to be salvaged and which may contain regulated refrigerants (CFCs, HCFCs, HFCs, PFCs and any blends of these) must certify safe transport methods to the DNR. See DNR Refrigerant Recovery Program.  
Sample Timber Sale Contract (for Private Land Timber Harvests)
A timber sale contract is the most important document involved in conducting a timber sale. A well-prepared contract ensures the understanding of all parties. Timber sale contracts define who, what, where, when, and how a timber sale will occur. Contracts will also spell out remedies in the event of a dispute. The sample contract provides a template of important items to consider for a successful harvest. The Wisconsin Woodland Owners Association, DNR, and UWEX developed the form. Use of the sample contract is voluntary. Woodland owners are encouraged to seek the assistance of a professional forester when they harvest timber. (A directory of DNR and Cooperating Foresters is available at http://dnr.wi.gov/forestry/private/.) Landowners may also wish to consult with an attorney regarding the terms of contracts.  
Sanitarian Registration

2005 Wisconsin Act 25 transferred responsibility for the credentialing and regulation of sanitarians from DHFS to DRL. Current authority for credentialing and regulation of sanitarians may be found in WIs. Stat.Ch.440 subch. XI . School Approval
Approval to become a postsecondary school or offer a new program in Wisconsin.  
School Program Approval
New and revised programs offered by schools subject to EAB oversight must be evaluated and approved to ensure they are consistent in quality, content, and length with generally accepted educational standards.  
Scientific Collectors Permit
Section 29.17 (now 29.614), Wis. Stats., provides the Department of Natural Resources with the authority for issuing Scientific Collectors Permits. Section NR 19.11, Wis Adm. Code, outlines the requirements for the permits. In general a Scientific Collectors Permit is required to collect or posses protected fish and wildlife specimens for scientific and/or educational purposes.

Securities Registration
A firm wanting to sell its securities is required to obtain an Order of Registration-Securities to offer or sell non-exempt or non-federally covered securities in Wisconsin.  
Seed Collection on DNR Land
Present law (NR 45.04) and (NR 45.13) prohibits the collection of plants and plant parts, including seeds, from Department land without a permit. Seed collection will be by permit only. Permits may be issued for restoration projects undertaken by a government agency or conservation organization. Commercial native plant nurseries may be permitted to harvest seeds for stock plants of known regional genetic source for seed and plant production purposes. These stock plants must be established in Wisconsin production beds.  
Seed Labeler License
License required to label agricultural, lawn, or vegetable seed. A labeler is whose name and address appears on the seed label or who opens seed containers with the intent to sell in Wisconsin.  
Self-Insured Liability Plans
Self-insured plan refers to a method, other than through the purchase of insurance, by which a provider may furnish professional liability coverage that meets the requirements of ch. 655, Wis. Stats.  
Seller of Checks
Businesses that receive money for transmission or sell or issue checks, including money orders, or travel checks. Does not apply to banks, credit unions, savings and loans or post offices. The purpose is to ensure that funds transmitted by the use of money orders and traveler checks are reasonably safe.  
Septage Servicing Business Licensing Regulations
Establishes standards for servicing of septage systems to protect public health and waters of the State. Licensing allows a person or business to collect and dispose of septage.  
Septage Servicing Operator Certification
Examination program for certification of operators of septage servicing vehicles pursuant to ss. 281.17(3)): Adm. Code NR 114.  
Set or Bank Pole License
A Set or Bank Pole License authorizes residents to use not more than five set or bank poles for taking, catching, or killing fish in inland waters.  
Sheep: Brucella Ovis-Free Certificate
Certifies that sheep flock is free of brucella ovis. Voluntary. Facilitates movement and sale of sheep.  
Shelter Care
A facility licensed to provide short-term, nonsecure residential care and physical custody of children pending court action.  
Sign, Outdoor - Installation Application and Permit
To regulate outdoor advertising signs located on private lands that are visible from interstates, federal aid primary highways, and the Great River Road.  
Single State Registration (SSRS)
Fulfills obligation of filing interstate operating authority and proof of liability insurance by interstate for-hire motor carriers.  
Site Assessor
Allows a person to conduct a site closure assessment required under s. Comm 10.734.  
Site Closure Approval
This provides approval for cleanup of contaminated soil and/or groundwater.  
Slat Net License
A Slat Net license authorizes a resident to take commercial fish using slat nets in certain portions of the Mississippi River.  
Snowmobile Commercial Dealer Certificate
Every person who is a snowmobile manufacturer, dealer, distributor, renter or any combination thereof, engaged in business in this State shall register with the Department and obtain a commercial snowmobile certificate.  
Snowmobile Registration
All snowmobiles must be registered with the Department of Natural Resources for either PUBLIC USE (a snowmobile used on marked trails, frozen waters, or any lands you have permission to operate on. Kitty-Kat, child-like snowmobiles, with 4 horsepower motor or less and operated on trails must be registered) and/or PRIVATE USE (a snowmobile used only on land owned or leased by the person or a member of the person’s immediate family.) There is not an agriculture registration for snowmobiles.

Definition of a snowmobile: "Snowmobile" means an engine-driven vehicle that is manufactured solely for snowmobiling, that has an endless belt tread and sled-type runners, or skis, to be used in contact with snow but does not include such a vehicle that is any of the following:
  • A vehicle that has inflatable tires.
  • A vehicle that is driven by a motor of 4 horsepower or less and that is operated in sanctioned races, derbies, competitions or exhibitions or only on private property.

Snowmobile Safety Instructor
Certificate available for people who have an interest in recreational safety and who are willing to donate a few hours of their time each year to teach safety education classes in their community. The Department of Natural Resources trains and certifies volunteers to teach about snowmobiling, and provides instructors with class materials and supplies. Becoming a volunteer instructor is a great way to help save lives, prevent injuries, meet people in your community, and positively influence the attitudes and actions of other resource users.  
Snowmobile Safety Student
Certificate is required by all snowmobile operators at least age 12 and those born on or after January 1, 1985 to operate in public areas. Certificate granted to individuals who complete the snowmobile safety course, which covers the principles of snowmobile safety. The purpose of the program is to reduce the potential for snowmobile accidents; to reduce the potential for conflict between snowmobilers, landowners, and other resource users; and to promote safe, responsible, and ethical use of the environment and natural resources.  
Snowmobile Trail Pass

Non-Resident Snowmobile Trail Pass Snowmobiles operated on public lands need to display valid Wisconsin Public Use Snowmobile registration or a valid Non-Resident Snowmobile Trail Pass and valid Non-Resident registration.

Snowmobiles in Wisconsin for more than 15 consecutive days are required to be registered in Wisconsin.

There are no requirements to show proof of residency for the snowmobile or the purchaser. Passes are not assigned or designated to a specific snowmobile or person. Passes are not transferable from one snowmobile to another.

 
Snowmobile, Municipal Certificate
Snowmobiles owned by the United States, another state or a political subdivision thereof shall either register their snowmobile with the Department or display their organization name in a visible manner on the exterior of the snowmobile.  
Social Worker
Certificate required for an individual to provide social work services without supervision, including psychotherapeutic activities as permitted in state administrative code.  
Social Worker, Advanced Practice
Certification required for a person to perform or evaluate and intervene in complex difficulties in psychosocial functioning. Intervention plans may include psychosocial evaluation, counseling of individuals, families, and groups; referral to community resources; advocacy; and facilitation of organizational change to meet social needs.  
Social Worker, Independent
Certification required for a person to evaluate and intervene in complex difficulties in psychosocial functioning. Intervention plans may include psychosocial evaluation, counseling of individuals, families, and groups; referral to community resources; advocacy; facilitation of organizational change to meet social needs.  
Social Worker, Independent Clinical
Certificate required for a social worker to provide social work services without supervision.  
Social Worker, Training Certificate
Certification required for a person to perform with a bachelor's degree in psychology, sociology, criminal justice or other human service program working as a social worker while attaining courses and experiences that will qualify him or her for the basic social worker certificate. The certificate valid for only 24 months.  
Soil and Plant Additive License
License required to sell or distribute soil and plant additives in Wisconsin. A soil and plant additive is any substance that will be applied to seeds, soil, or plants to promote or sustain plant growth or soil fertility.  
Soil Erosion Inspector Certification
Allows a person to inspect one- and two-family dwellings for the purpose of administering and enforcing s. Comm 21.125 on soil erosion control standards and public buildings and places of employment for the purpose of administering and enforcing ch. Comm 60.  
Soil Scientist, Professional
License required by a person who performs by reason of his or her knowledge of mathematics, the physical sciences and the principles of soil science acquired by professional education and practical experience, is qualified to engage in the practice of professional soil science.  
Soil Tester Certification
Allows a person to conduct soil evaluations relative to the discharge or disposal of liquid domestic wastes into the soil.  
Solid Waste Collection and Transportation/Recyclables Transportation License
License required to operate a solid waste collection and transportation service or a recyclables transportation service in Wisconsin. For businesses such as garbage collection.  
Solid Waste Landfill Facility License
License required to construct and operate a solid waste landfill or surface impoundment including the following types: Construction and Demolition Waste Small Size, Construction and Demolition Waste Intermediate Size, Municipal Solid Waste, Industrial Solid Waste, and One Time Disposal.  
Solid Waste Landfill Operator Certification
Examining program for certification of landfill operators pursuant to S. 144.435.  
Solid Waste Non-Landfill Facility License
License required to construct and operate a solid waste facility of the following types: Storage, Transfer, Incineration, Air Curtain Destructors, Processing, Wood Burning, Composting, and Municipal Solid Waste Combustors.  
Something Special from Wisconsin Trademark Use Permit
Permit required for businesses to use the Something Special from Wisconsin trademark.  
Special Education Program Aide
Required for individuals employed as instructional aides for children with identified exceptional education needs, including special education classroom aides and special education transportation aides.  
Special Events Recreational Use License
This license is required for anyone holding a competition or other non-routine event in a state park, forest, or trail or on other Department of Natural Resources lands. Examples include foot, bike, or canoe races; horse endurance events; Civil War encampments outside designated campgrounds; marathons; hot-air balloon rallies; and events that sell food or beverages to the public. Routine activities, DNR-sponsored events, and events held entirely on roads and waterways normally do not require a license.  
Speech-Language Pathologist
License required for a person to perform or apply principles, methods or procedures of prevention, identification, evaluation, consultation, intervention, instruction or research related to speech, language, cognition or swallowing or any abnormal condition involving speech, articulation, fluency, voice, verbal or written language, auditory comprehension, cognition or communication or oral, pharyngeal or laryngeal sensorimotor competencies.  
Sport Trolling
Required to guide, direct or assist other persons in sport trolling for trout and salmon in and upon the outlying waters of Lake Michigan, Green Bay and Lake Superior.  
Stage 2 Vapor Recovery Training Certificate
Training for employees of gasoline dispensing facilities required in s. NR 420.045(5).  
State Park Vehicle Admission Sticker, Annual
Sticker entitles the vehicle to which it is attached to enter state parks without paying daily vehicle admission charges. A resident sticker is for a vehicle with Wisconsin license plates; a non-resident sticker is for a vehicle with licence plates from anywhere else.  
State Trail Pass
A state trail pass is required for anyone age 16 or older who rides a bicycle or horse, roller skates, or skis on a trail which requires the pass.  
Storage Tank Registration (Aboveground) - Flammable/Combustible/Hazardous Liquids
Aboveground tanks in Wisconsin that have stored or currently store petroleum or CERCLA regulated substances must be registered.  A separate form is needed for each tank.

Sturgeon Spearing License
A Sturgeon Spearing License is required for both residents and nonresidents and allows spearing of Rock or Lake Sturgeon in Wisconsin.  
Substitute Teacher License
The substitute license allows the holder to accept long-term employment (20 or more consecutive days in any one assignment) in public schools in the area in which he or she received institutional endorsement. The holder may also accept short-term employment (fewer than 20 consecutive days in any one assignment) in any grade or subject  
Substitute Teacher Permit
The Substitute Permit allows the holder to accept short-term substitute teaching assignments (fewer than 20 consecutive days) in public schools when no regularly licensed teachers/substitute teachers are available.

 
Surplus Lines Agent
License required for a person to be an agent or broker of surplus lines insurance, which is insurance placed with unauthorized insurers through surplus lines agents. Agents must have an intermediary license prior to applying for a surplus lines license.  
Swimming Pools, Public
A public swimming pool includes but is not limited to a pool serving or installed for the state or any political subdivision of the state; a pool serving or installed at a motel, hotel, tourist rooming house, bed and breakfast establishment, campground, camp, club, association, housing development or school, or a religious, charitable or youth organization; a mobile pool; and a pool at an educational or rehabilitative institution. Included in the meaning of a"public swimming pool" are buildings, equipment and appurtenances, irrespective of whether or not a fee is charged for their use.  
Take-Over Offer Registration
To provide that any take-over offer for a Wisconsin target company provides stockholder-offerees with full disclosure regarding the offer, is made to all stockholder-offerees on equal terms, and the offeror has not committed violations of the take-over or securities laws.

Tank System Installer, Underground
Allows a person to install an underground tank system which is to hold flammable or combustible liquids.  
Tank System Liner
Allows a person to line a tank system which has held or will hold flammable or combustible liquids.  
Tank System Removers and Cleaners
Allows a person to remove or clean a tank system which has held flammable or combustible liquids.  
Tank System Tightness Tester
Allows a person to conduct the tightness testing of a tank system that has held or will hold flammable or combustible liquids  
Tanning Device

A permit to operate a tanning device or devices available to the public for the purpose of artificial light skin tanning, including those offered for use as part of a membership or premium offer in a health club, condominium ownership, apartment complex activity center, hotel or motel room rental. No person may operate a tanning facility without a permit issued by the department.

 
Tattooing and/or Body-Piercing Establishment
A permit to operate any premises where a tattooist applies a tattoo to another person, or a premises where a body piercer performs body piercing, or both.  
Tattooist and/or Body Piercer
A certification for a person who applies a tattoo to another person, and/or a person who performs body piercing.  
Taxidermist
Required to mount or preserve the carcass of any wild animal for consideration. Taxidermist license also includes the privileges extended by a Class A Fur Dealer License.  
Teaching Location(s)
Approval of a site where a school will offer instruction.  
Telecommunications Utilities (New Alternative) Certificate of Authority
To authorize such an entity to transact business in Wisconsin under s.196.203 (2), Stats. Entities covered under this section include cable television communications service providers (Ch. PSC 171, Wis. Adm. Code), telecommunications resellers (including alternative operator service providers). (Ch. PSC 168, Wis. Adm. Code), and any other telecommunications provider if the commission finds that the service offered by the telecommunications provider is available from other telecommunications providers within this state directly or indirectly to the public. Examples of other alternative telecommunications providers requiring PSC certification include competitive local exchange companies (CLECs) and video distance learning network providers. Telecommunications providers not subject to PSC certification as a condition of transacting business, but which may be subject to other PSC regulations include pay telephone service providers, shared tenant service providers and commercial mobile radio service providers.  
Telecommunications Utilities (New) Certificate of Authority
To authorize such an entity to transact business in Wisconsin under s.196.59 (3), Stats.  
Telephone Solicitor Registration
Registration required to make telephone solicitations to a residential customer in Wisconsin.  
Time-Share Salesperson
License required for a person to perform as an individual other than a person licensed as a real estate broker or salesperson under s. 452.09, Wis. Stats., who is employed by a licensed broker to sell or offer or attempt to negotiate an initial sale or purchase of a time share but who may not perform any other acts authorized by Chapter 452, Wis. Stats., to be performed by a broker or salesperson.  
Title, Vehicle - New Wisconsin resident
A vehicle Certificate of Title is proof of vehicle ownership. When you become a Wisconsin resident, you must register your vehicle and get Wisconsin license plates. If you submit your title when you apply for Wisconsin license plates, you will be issued a Wisconsin title. If you submit only a registration card with your application, you will be issued Wisconsin license plates and a registration card, which will allow you to operate the vehicle legally, but the registration card is not good for selling the vehicle. Title fee is required in both cases as titling is prerequisite to registration.  
Tourist Rooming House
All lodging places and tourist cabins and cottages, other than hotels and motels, in which sleeping accommodations are offered for pay to tourists or transients. It does not include private boarding or rooming houses not accommodating tourists or transients, or bed and breakfast establishments.  
Trade Names and Trademarks
Registration of a trade name or trademark showing the date of use by the registering party, effective for ten years.  
Trammel Net License
Required to use a trammel net on the Wisconsin-Iowa Boundary Waters.  
Trapping
Issued to residents to authorize the use of traps for trapping fur-bearing animals. Special permits are required in addition to license to trap otter, fisher and bobcat.  
Tree City USA Certification
Tree City USA is a certification that a city, village or township has met the four Tree City USA standards established by the National Arbor Day Foundation to recognize local governments for their management of their community’s trees.  
Tree Line USA certification
Tree Line USA is a certification that an investor-owned, cooperative, or municipal utility company has met the three Tree Line USA requirements established by the National Arbor Day Foundation. It recognizes utilities that demonstrate management practices which protect and enhance urban forests.  
Trip permit, Multiple - for Oversize/Overweight Loads
Permit to transport loads that cannot be transported within normal legal sizes and weights. The multiple trip permit allows unlimited trips during the months specified in the permit.  
Trip Permit - Special Transportation Need
Issued to motor carriers engaged in interstate or intra- state operations in instances of special transportation need such as response to storm damage and drought. The fee may be waived.  
Trip Permit, Single - for Oversize/Overweight Loads
Permit to transport non-divisible loads which cannot be transported within normal legal sizes and weights.  
Uniform Commercial Code (UCC)
To process statements of business indebtedness, maintain a file of indebtedness statements and respond to questions.  
Utility Contractor Registration
Allows a person to install or modify water services, private water mains, sanitary building sewers, storm building sewers, or private interceptor main sewers.  
Utility Waterway Crossing Permit - Waterway & Wetland Permit
A permit or written approval is required to cross a waterway with a pipeline, cable or other utility line, where construction involves the plow-in method, trench excavation or dredging. Applies to anyone who wants to install a pipeline, cable or other utility line across or underneath a waterway.  
Vegetable Contractor License
Original or Renewal license for a contractor who procures vegetables from producers for use in processing must hold a vegetable contractor license. "Contractor" includes a food processing plant or a handler who procures vegetables for sale to a processor.  
Vending of Food

A permit is required for any vending machine commissary and any self-service food or drink vending machine offered for public use. Vending machines that dispense the following do not require a license: bottled, prepackaged or canned soft drinks; candy; gum; nuts; nut meats; cookies; crackers; pastry items which have a pH level of 4.6 or below or a water activity (aw) value of 0.85 or less under standard conditions; or prepackaged Grade A pasteurized milk or milk products.

 
Veterinarian
License required for a person to perform and examine into the fact or cause of animal health, disease or physical condition, or to treat, operate, prescribe or advise for the same, or to undertake, offer, advertise, announce, or hold out in any manner to do any of said acts, for compensation, direct or indirect, or in the expectation thereof.  
Veterinarians: Approval to Test
Veterinarians must be approved to test animals for interstate shipment, and to administer certain tests and vaccines.  
Veterinary Technician
Certificate required for a person to perform work under the direct supervision of a licensed veterinarian specific veterinary tasks within the scope of the professional guidelines for the occupation.  
Viatical Settlement Broker
License required for a person or entity to act as a viatical settlement broker, who, for a fee, commission, or other valuable consideration, offers or attempts to negotiate settlements between a life insurance policyholder or certificate holder and one or more viatical settlement providers.  
Viatical Settlement Provider
License generally required for a person to act as a viatical settlement provider, which is an individual, partnership, or corporation that pays a viatical settlement.  
Visible Emission Certification
Visible emission certification allows an individual to read visible emissions from a smoke stack or fugitive emissions.  
Warranty Plan
Certificate of authority required for a person to offer a warranty plan, by which a third party provides repair, replacement, or maintenance services in connection with the sale of goods from seller to buyer (e.g., automobiles, homes, appliances, etc.).  
Water Carrier Authority - Certificate of Public Convenience and Necessity
Certificate enables common carriers of passengers or property by water with fixed endpoints. No person shall operate as a common carrier of passengers or property by water except in accordance with terms and conditions set forth in certificate of public convenenience and necessity.  
Water Level Order - Waterway & Wetland
A Department of Natural Resources (DNR) Order may be issued to establish a water level or flow requirement on a navigable waterway. Any person may petition or request DNR to establish a water level or flow requirement for a navigable waterway. DNR may also initiate action on its own motion to regulate levels or flows. Water level or flow requirements may be established by a DNR order, or as part of a permit or approval for a dam.  
Water Ski Platform or Water Ski Jump Permit - Waterway & Wetland Permit
A permit is required for the placement of any structure in a waterway. A water ski platform or water ski jump may be placed without a permit if certain conditions are met. If these conditions are not met, a miscellaneous structure permit is required for a water ski platform or water ski jump. Applies to anyone who wants to place a water ski platform or water ski jump into a waterway.  
Water Well, Driller

A license is necessary for drilling drinking water wells. Drinking water wells can be for industrial, municipal, or private residential use.
