Description:
Recertification for those providers receiving payment by Medicaid for services provided.  
License Type:
Renewal  
Duration:
One to four (4) years depending on your provider type.  
Fees:
No fees required.  
Prerequisites:
Already certificated to receive payment by Medicaid for services provided. The provider must continue to meet the specific requirements.

If you are an individual who wants to find out if you are eligibile for Medicaid to pay for your medical services, contact your county or tribal department of social or human services.

Recertification information is sent to each provider.  If you do not receive your recertification packet, contact Provider Services at telephone 1-800-947-9627.
