Detailed Information for : Funeral Director
License required for a person to perform services in conjunction with conducting, or holding himself or herself out, in whole or in part, as being engaged in embalming or otherwise preparing for the burial or disposal, or directing and supervising the burial or disposal, of dead human bodies as defined in s. 445.01, Wis. Stats.  
License Type:
Two years. Renewal due date is December 31 of each odd-numbered year.  
Valid Wisconsin credential or license (see initial application information).  
Application Form(s):
Renewal notice is mailed 6 weeks before the renewal due date. For example, you receive a renewal notice on November 15 (of odd-numbered years) to be renewed by deadline date of December 31 (of odd-numbered years). Special renewal requirements are listed on the back of the renewal form.
