Detailed Information for : Social Worker, Advanced Practice

Description:	Certification required for a person to perform or evaluate and intervene in complex difficulties in psychosocial functioning. Intervention plans may include psychosocial evaluation, counseling of individuals, families, and groups; referral to community resources; advocacy; and facilitation of organizational change to meet social needs.
License Type:	Renewal
Duration:	Two years. Renewal due date is June 30 of each odd-numbered year.

Prerequisites:	Valid Wisconsin credential or license (see initial application information).
Application Form(s):	Renewal notice is mailed 6 weeks before the renewal due date. For example, you receive a renewal notice on May 15 (of an odd-numbered year) to be renewed by deadline date of June 30 (of an odd-numbered year). Special renewal requirements are listed on the back of the renewal form.
