Detailed Information for : Nurse, Licensed Practical
Description:
License required for a person to perform simple acts of nursing care or acts under specific direction of a qualified professional. The license holder is referred to as an L.P.N.

Duration:
Two years. Renewal due date is April 30 of each odd-numbered year.

Valid Wisconsin credential or license (see initial application information)  
Application Form(s):
Renewal notice is mailed 6 weeks before the renewal due date. For example, you receive a renewal notice on March 15 (of an odd-numbered year) to be renewed by the deadline date of April 30 (of an odd-numbered year). Special renewal requirements are listed on back of renewal form.
