$120.00 Credential (prorated for partial cycle).

| Prerequisites: |
The applicant shall provide proof of
1) passing the competency exam by the national board of boiler and pressure vessel inspectors and
2) either
(a) a degree in engineering from an accredited college or university and at least 1,000 hours of experience in the design, construction, operation, or inspection of boilers or pressure vessels; or
(b) an associate degree in mechanical engineering from an accredited college or university and at least 1,000 hours per year for at least 2 years of experience in the design, construction, operation, or inspection of boilers or pressure vessels, or
(c) at least 1,000 hours for at least 3 years of experience in the design, construction, operation, or inspection of boilers or pressure vessels.

| Required Documentation: |
Applicant required to submit a signed and completed application form, a copy of one of the following documents:
1. A letter stating that you have passed the competency examination;
2. A copy of the Certificate of Competency; or
3. A copy of a National Board commission.
If competency exam or certificate of competency was done in another state, attach a copy of the official transcripts from school where you hold an engineering degree or mechanical technology associate degree.
